For patients with $80 Evaluation Cards, these cards may be combined with patient history forms discount offer. You will pay $70 with both an $80 Evaluation Card and patient history forms.
If you are a new patient in the armed forces or a veteran, your price will be $70 with your patient history forms or, if a renewal, $40 with your patient history forms.
